Welcome to Fullington Physical Therapy ("we," "our," or "us"). By accessing and using our telehealth services, you (the "patient" or "user") agree to comply with and be bound by the following Terms of Service. These terms govern your use of our telehealth platform, including any services, features, or content available through it.

Please read these Terms carefully before using our services. If you do not agree to these terms, you should not use our telehealth services.

1. Services Provided

Fullington Physical Therapy offers telehealth services designed to deliver physical therapy consultations and care through online platforms. Our services may include, but are not limited to:

Virtual physical therapy consultations and assessments

Personalized treatment plans

Exercises and educational materials

Follow-up appointments via video or other telecommunication methods

All services are provided by licensed physical therapists.

2. Eligibility

To use our services, you must:

Be at least 18 years of age or have a parent/legal guardian consent to the treatment if under 18. Be physically located within a jurisdiction where we are legally permitted to provide telehealth services.Provide accurate and complete information regarding your medical history, conditions, and any other relevant health information.

You may be required to complete an intake form and agree to a telehealth consent before receiving services.

3. Telehealth Consultations

Our telehealth consultations are conducted via secure video or phone platforms. You agree to:

Participate in telehealth sessions in a private, quiet, and safe environment.Ensure that your technology (computer, internet connection, webcam, microphone, etc.) is functioning correctly prior to each session.Maintain confidentiality during consultations and not record or share any sessions without express consent.

While we strive to provide high-quality care, telehealth consultations may have limitations. If, during a session, we determine that an in-person visit is necessary, we will advise you accordingly.

4. Privacy and Confidentiality

We are committed to maintaining your privacy and confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state laws. Your personal and medical information will not be shared with third parties except as necessary for your treatment or as required by law.

By using our services, you consent to the collection and use of your health information for the purpose of providing physical therapy care.

5. Fees and Payment

Fees: Fullington Physical Therapy charges a fee for its telehealth services, which may vary depending on the service type. The cost of your treatment will be discussed during the scheduling process.

Payment: Payment for services is required at the time of the session or as otherwise agreed upon. Payments are accepted through IntakeQ

Insurance: If you intend to use insurance, it is your responsibility to confirm your coverage and determine if telehealth physical therapy services are covered. We may provide a superbill for you to submit to your insurance carrier if applicable.

Refunds: Refunds may be issued in cases of billing errors or cancellations made in accordance with our cancellation policy (see below).

6. Cancellation and No-Show Policy

Cancellations: You may cancel or reschedule your appointment up to [24] hours before your scheduled session without penalty. Cancellations made less than [24] hours before the session will incur a $25 fee.

No-Shows: If you miss an appointment without prior notice, you will be charged a no-show fee of $25. Repeated no-shows may result in the termination of services.

7. Responsibilities of the Patient

By using our services, you agree to:

Provide accurate and up-to-date health information to your physical therapist.

Follow the prescribed treatment plan and exercise regimen to the best of your ability.

Notify Fullington Physical Therapy immediately if you experience any difficulties or changes in your condition.

Ensure you have the necessary technology (internet connection, webcam, etc.) for telehealth services.

8. Limitations of Liability

Fullington Physical Therapy makes no guarantees about the effectiveness of treatment, as outcomes vary depending on individual conditions. You acknowledge that:

Telehealth services may have limitations, including technological issues, and may not be appropriate for all conditions.We are not liable for any damages or injury resulting from the use of telehealth services, including technical failures or communication interruptions.Our liability is limited to the amount paid for the specific telehealth service in question.

9. Termination of Services

We reserve the right to discontinue or suspend services at our discretion, including but not limited to:

Violations of these Terms of Service.Disruptive, abusive, or inappropriate behavior.Failure to comply with recommended treatment plans.

You may terminate services at any time by providing notice to Fullington Physical Therapy.

10. Governing Law and Dispute Resolution

These Terms of Service shall be governed by and construed in accordance with the laws of Missouri. Any disputes arising from these terms will be subject to binding arbitration, unless otherwise required by law.

11. Modifications to Terms of Service

We may update or modify these Terms of Service from time to time. Any changes will be posted on this page with an updated effective date. By continuing to use our services after such changes are made, you agree to the updated terms.
